 Library Instruction Session Request

Faculty or the person assigned to the course may request a library instruction session by using this form or by contacting the librarian directly.  Lectures may also be requested by student services departments (excluding U100 classes). 

Due to the heavy demand for library instruction, requests must be submitted at least 1 week prior to the requested lecture date.

The library cannot accept instruction requests that do not allow one week advanced notice.

Unless otherwise arranged, all library instruction lectures are scheduled in instruction rooms located in the University Library.


The submitted request will be sent directly to the librarian responsible for your discipline.  The fields on the request form marked with
** are required.   Please use a separate form for each session you request.

Date and Time Requested for Library Instruction Requests must be submitted at least one week prior to the requested lecture date.
Requests that do not allow one week advance notice will not be accepted. Indicate alternate lecture dates in case the librarian cannot accommodate your 1st choice date.

IMPORTANT NOTE
When you click on "SUBMIT REQUEST", this form is emailed directly to the librarian responsible for your discipline and you will receive an email note acknowledging that your REQUEST was submitted.
A confirmation from the librarian indicating the date, time and location will be your notification that the class session you requested is actually scheduled.

Requestor should notify the librarian if the class cannot arrive at the scheduled lecture time. The lecture will be considered cancelled if the class does not arrive within 15 minutes of the scheduled time.
